This course will assist clinicians with their evidence-based-decision-making and dialogue with patients when discussing matters of causality. From a public health and epidemiological perspective, this course will provide participants with the tools to evaluate cause and effect relationships and to better understand the principles of determining causality.
Dental implants as we know them today were invented in 1952 by a Swedish orthopedic surgeon named Per-Ingvar Brånemark. Today, they are considered the standard of care for prosthetic replacement of missing teeth in dentistry. A dental implant is a surgical fixture that is placed into the jawbone and allowed to fuse with the bone over the span of a few months. The dental implant acts as a replacement for the root of a missing tooth. In turn, this "artificial tooth root" serves to hold a replacement tooth or bridge. Having a dental implant fused to the jawbone is the closest thing to mimicking a natural tooth because it stands on its own without affecting the nearby teeth and has great stability. The process of fusion between the dental implant and jawbone is called "osseointegration." Most dental implants are made of titanium, which allows them to integrate with bone without being recognized as a foreign object in our body. Over time, technology and science have progressed to greatly improve the outcomes of dental implant placement. Today, the success rate for dental implants is close to 98%.
As gingivitis progresses (chronic gingivitis), various complications may arise. The affected person may develop receding gums or areas where the root of the tooth becomes uncovered by the shrinking, diseased gums. Deep pockets may develop around the teeth that trap food, plaque, and debris. If gingivitis advances to periodontal disease, the person may lose gum tissue or bone around the teeth and the teeth may become loose or fall out. These changes can develop either very slowly or very rapidly and can affect either a few teeth or the entire mouth. If oral hygiene is almost never done or if the person becomes immune-compromised, acute necrotizing ulcerative gingivitis may develop (ANUG, formerly termed trench mouth). This is a painful condition where infected gums swell, ulcerate, and slough off dead tissue. Dental Implants Clinic Nurident- UK patient from London

A dental implant is a titanium post (like a tooth root) that is surgically positioned into the jawbone beneath the gum line that allows your dentist to mount replacement teeth or a bridge into that area. An implant doesn't come loose like a denture can. Dental implants also benefit general oral health because they do not have to be anchored to other teeth, like bridges.
Our offices work to bridge the gap between the dental and medical communities. When left untreated, dental emergencies can lead to Hospital ER and Urgent Care Center visits resulting in risk of infection, extended healing times and excessive medical bills. Our goal is to run an efficient, walk-in practice that exclusively treats dental emergencies, so you can get out of pain quickly and back to your daily routine. Dental Clinic
Most dental implant procedures involve less discomfort than a tooth extraction. When we see patients for follow up visits, most of them tell us how surprised they were with how little discomfort they had! (TRUE!) Our office is equipped with doctors and a staff trained to safely provide conventional methods of anesthesia as well as a variety of intravenous sedation anesthesia options. Operative procedures will seem both pleasant and brief when effective local and IV anesthesia is chosen. For an implant to become permanently stable, the body must grow bone to the surface of the implant (osseointegration). Based on this biologic process, it was thought that loading an implant during the osseointegration period would result in movement that would prevent osseointegration, and thus increase implant failure rates. As a result, three to six months of integrating time (depending on various factors) was allowed before placing the teeth on implants (restoring them).[8]

Dentures, removable replacements for missing teeth, come in two types: partial and complete dentures. Dental implants and bridges are suitable alternatives to dentures. Though it usually takes a while to get used to eating and speaking with new dentures, denture adhesives can improve the stability of the denture and provide a sense of security for the denture wearer. If a wire in your braces breaks off or starts poking you in the cheek, this is similarly an emergency that can wait until the next day. To stop the wire from poking you, try using a pencil eraser to bend it back against your teeth. Do not cut the wire; it may accidentally get stuck in your tongue, or you may swallow it. Cover the end of the wire with some orthodontic wax to prevent it from rubbing on your cheek. Try to stick to soft foods until you have your braces repaired so that you don't cause more extensive damage.
At the other end of the cost spectrum are over-the-counter strips and trays that you can buy at your local pharmacy or grocery store. These products can range from $20 to $100, making them an attractive option for those looking for a bit of smile enhancement without the higher cost of professionally administered whitening. However, it’s important to keep in mind that results can vary drastically when using these cheaper, low-concentration peroxide whiteners.

The term “whitening” on the other hand, refers to restoring a tooth’s surface color by removing dirt and debris. So technically speaking, any product that is used to clean the teeth (like a toothpaste) is considered a whitener. Of course, the term whitening sounds better than bleaching, so it is more frequently used — even when describing products that contain bleach. Teeth Whitening Dentist Camden

Internal staining of dentine can discolor the teeth from inside out. Internal bleaching can remedy this on root canal treated teeth. Internal bleaching procedures are performed on devitalized teeth that have undergone endodontic treatment (root canal treatment) but are discolored due to internal staining of the tooth structure by blood and other fluids that leaked in. Unlike external bleaching, which brightens teeth from the outside in, internal bleaching brightens teeth from the inside out. Bleaching the tooth internally involves drilling a hole to the pulp chamber no more than 2mm below the gingival margin, cleaning any infected or discoloured dentine, sealing, and filling the root canal with gutta-percha points, cleaning the inside of the canal using etchant and placing a peroxide gel or sodium perborate tetrahydrate into the pulp chamber so they can work directly inside the tooth on the dentine layer.[citation needed] In this variation of whitening the whitening agent is sealed within the tooth over a period of some days and replaced as needed, the so-called "walking bleach" technique.[citation needed]. A seal should be placed over the root filling material to minimise microleakage. There is a small risk of external resorption. Dental Clinic

Make a sea salt solution. Dissolve a small amount of sea salt in a cup of warm water. Swish a sip of the solution in your mouth for 30 seconds and spit it out. Repeat several times. Salt water will reduce swollen gums, gum bleeding and will also decrease the swelling caused by infection. However, if the infection has advanced to an abscess, then you will need antibiotics. Add this mouth rinse to your twice-daily brushing routine.

After scaling and root planing, if the gum tissue is not fitting snugly around the tooth and you can't keep the deep pocket area clean, you may be a candidate for periodontal pocket reduction or flap surgery. By folding back the gum tissue, your dentist or periodontist can remove infectious bacteria and smooth areas of damaged bone, allowing the gum tissue to reattach to healthy bone.

Doxycycline may be given alongside the primary therapy of scaling (see § initial therapy).[62] Doxycycline has been shown to improve indicators of disease progression (namely probing depth and attachment level).[62] Its mechanism of action involves inhibition of matrix metalloproteinases (such as collagenase), which degrade the teeth's supporting tissues (periodontium) under inflammatory conditions.[62] To avoid killing beneficial oral microbes, only small doses of doxycycline (20 mg) are used.[62] Invisalign review for Bishopsgate Dental Care in London

If you are suffering from any of the symptoms listed above, it is important that you visit and consult with your dentist to determine the severity of infection and the proper course of action to take to combat the issue.Gum Infection TreatmentsAs soon as you notice a gum infection, you should consult with a dentist. Once gum infection is diagnosed, you will have numerous treatment options that will vary based on the severity of the gum infection. Here are some common methods of treatment for gum infection:

Gum disease, worn enamel, cavities, and exposed roots. Individuals with gum disease or teeth with worn enamel are generally discouraged from undergoing a tooth-whitening procedure. Cavities need to be treated before undergoing any whitening procedure. This is because the whitening solutions penetrate into any existing decay and the inner areas of the tooth, which can cause sensitivity. Also, whitening procedures will not work on exposed tooth roots, because roots do not have an enamel layer. Dentist In Central London East London London

Whitening toothpastes are different to regular toothpastes in a way that they contain a higher content of abrasives and detergents to fight off tougher stains. Most contain low concentrations of carbamide/hydrogen peroxide (bleaching agents) – this lightens the colour of teeth. These whitening toothpastes make teeth one to two shades lighter.[25] Toothpastes (dentifrices) which are advertised as "whitening" rarely contain carbamide peroxide, hydrogen peroxide or any other bleaching agent.[26] Rather, they are abrasive (usually containing alumina, dicalcium phosphate dehydrate, calcium carbonate or silica), intended to remove surface stains from the tooth surface.[26] Sometimes they contain enzymes purported to break down the biofilm on teeth.[26] Unlike bleaches, whitening toothpaste does not alter the intrinsic color of teeth. Excessive or long term use of abrasive toothpastes will cause dental abrasion, thinning the enamel layer[26] and slowly darkening the appearance of the tooth as the dentine layer becomes more noticeable. An alternative method of whitening at home, which competes with the use of a whitening paste, is the use of a patent whitening mouthwash. The mouthwash shows marginally better efforts at whitening compared to the use of paste.[27] Dental Implant Specialist London - Forest & Ray Dental

Soft tissue reflection: An incision is made over the crest of bone, splitting the thicker attached gingiva roughly in half so that the final implant will have a thick band of tissue around it. The edges of tissue, each referred to as a flap are pushed back to expose the bone. Flapless surgery is an alternate technique, where a small punch of tissue (the diameter of the implant) is removed for implant placement rather than raising flaps.

Scaling and root planing. This is a deep-cleaning, nonsurgical procedure, done under a local anesthetic, whereby plaque and tartar from above and below the gum line are scraped away (scaling) and rough spots on the tooth root are made smooth (planing). Smoothing the rough spots removes bacteria and provides a clean surface for the gums to reattach to the teeth. Scaling and root planing is done if your dentist or periodontist determines that you have plaque and calculus (hardened plaque, also called tartar) under the gums that needs to be removed. Once you are done, you can rinse the gel off your teeth and clean the light and the dental trays. The suggested treatment plan for the best results is 5 to 15 minutes per day, but it is not clear how many treatments should be completed in total. It also isn’t clear how many treatments this kit can give. Having said that, with results seen as quickly as after the first use, you probably won’t run out too quickly. Day in the life of a Dentist

Our mouths are full of bacteria. These bacteria, along with mucus and other particles, constantly form a sticky, colorless “plaque” on teeth. Brushing and flossing help get rid of plaque. Plaque that is not removed can harden and form “tartar” that brushing doesn’t clean. Only a professional cleaning by a dentist or dental hygienist can remove tartar. Dental Implants on the NHS WARNING

Your dental team will remove all plaque and tartar from your teeth. You will also be shown how to remove plaque successfully yourself, cleaning all the surfaces of your teeth thoroughly and effectively. This may take a number of sessions with the dental team. A good oral care routine at home with brushing and interdental cleaning is the most important thing you can do to help prevent gum disease getting worse.

Both dental plaque and tartar are filled with harmful bacteria, and if they aren't removed from teeth, they will begin to irritate the gums and cause gingivitis. If left untreated, gingivitis will often extend from the gums to the bone and lead to periodontitis. When the underlying bone gets infected, it will start to recede away from the teeth and form deep gum pockets. These pockets collect plaque and bacteria as they are very difficult to keep clean, and more bone loss occurs. As periodontal disease progresses into later stages and more bone tissue is lost, the teeth may eventually become loose and fall out. Emergency Dentist Camden

Have a friend call the dentist immediately while you focus on the lost tooth. Always lift up the lost tooth by the crown, rather than the root, since you do not want to introduce any bacteria or other pathogens to the tooth root. If there is any debris or dirt on the tooth, run it under some clean, cold water, but do not dry it on anything. Push it back into its socket, making sure that it is facing the right way, and bite down slowly to keep it in place until you reach the emergency dentist. If you cannot easily get the tooth back into its socket, store it in a container of milk. Pharmacies also sell a product called Save-a-Tooth that you can place a lost tooth in if you happen to have it on hand.

If, as a result of an inadequate cleaning technique, plaque deposits are left on the teeth, these will become mineralised and turn into hard, fixed deposits (calculus, commonly known as tartar) that cannot be removed with a toothbrush. Your dentist or periodontist will identify these tartar deposits during your regular dental inspection and remove them as part of a professional clean. After the deposits are removed, the teeth are polished with special pastes and cups to create a smooth surface that is less likely to accumulate plaque. Best Dentist Implants London

The over-the-counter products are weaker. That means less active whitening but also less risk to the gums, should the whitening agent come in contact. "All of the products are safe if used as directed," Messina says. Still, they can increase sensitivity of teeth and they can irritate gum tissue. Welcome to Dental Care at Crystal Park! We are dedicated to providing you with high quality, individualized care for your oral health needs. The cheapest and most convenient of the teeth whitening options, over-the-counter bleaching involves the use of a store-bought whitening kit, featuring a bleaching gel with a concentration lower than that of the professionally dispensed take-home whiteners. The gel is applied to the teeth via one-size-fits-all trays, strips or paint-on applicators. In many cases this may only whiten a few of the front teeth unlike custom trays that can whiten the entire smile.

In this category, the product that gets the best reviews we spotted is the Go Smile Super White Snap Packs (Est. $60 for 14 treatments). Go Smile contains 14 tubes filled with gel that you squeeze onto your teeth. Reviews.com, which makes Go Smile their top pick in teeth whiteners, describes it as very easy to use, saying it's "quick to apply and totally wearable." In their testing, it lightened the testers' teeth three shades in a week's time. You use it twice a day for at least 20 minutes -- overnight for the evening treatment. In cases where gingivitis has led to periodontal disease and there are deep pockets that are difficult to clean, the patient may require deep scaling and root planing to clean teeth that are surrounded by deep pockets. They may need surgical treatment to gain access to all the tooth surfaces for a thorough cleaning. This surgical procedure is called flap surgery and can be combined with a pocket-reduction surgery to make the areas around the teeth easier for the patient to clean with brushing and flossing. This procedure consists of numbing the gums and then lifting them back to clean the teeth and sometimes reshape the bone. The gums are then repositioned around the teeth so there aren't the deep pockets that existed before treatment.

As with any toothache, you can keep abscess pain under control with cold compresses, salt water rinses, and sticking to soft foods. Your dentist may prescribe antibiotics for you to take before you come into the office; this often helps alleviate pain as it helps your body fight off the infection. Abscesses are often treated with a root canal procedure. If the tooth is badly infected, an extraction is an affordable treatment option. London Community Dental Alliance Low-cost Dental Clinic
Tooth whitening toothpastes. Because they're mildly abrasive, every toothpaste helps remove stains from teeth. Whitening toothpastes, however, also contain chemicals or polishing agents that help scrub stains from teeth without the aid of a bleaching agent. Tooth-whitening toothpastes are relatively inexpensive and brighten teeth by about one shade. Some whitening toothpastes contain peroxides, but they aren't left on the teeth long enough to have a whitening benefit.
Your first step in treating periodontitis is a conservative, nonsurgical treatment called scaling and root planing (SRP). A dentist or dental hygienist provides this treatment by scraping and removing the plaque and tartar off of your teeth and root surfaces by scaling, and then smoothing away any roughness on the roots to prevent bacteria from gathering again. According to Nevada-based dentist, Dr. Maria Perrone, SRP may take more than one visit and a local anesthetic can be used to prevent any discomfort. After this process, the gums will heal and reattach themselves to the healthy, clean surfaces of the teeth. Within a few weeks, your dentist will evaluate your healing and decide if further treatment is necessary.
Often, a second step of the procedure is necessary to uncover the implants and attach extensions. These temporary healing caps, along with various connecting devices that allow multiple crowns to attach to the implants, complete the foundation on which your new teeth will be placed. Your gums will be allowed to heal for a couple of weeks following this procedure. Cosmetic Dental Surgery - London Smile Dental Practice W1

Among the newest whitening products available are whitening rinses. Like most mouthwashes, they freshen breath and help reduce dental plaque and gum disease. But these products also include ingredients, such as hydrogen peroxide in some, which whiten teeth. Manufacturers say it may take 12 weeks to see results. You just swish them around in your mouth for 60 seconds twice a day before brushing your teeth. However, some experts say that rinses may not be as effective as other over-the-counter whitening products. Because a whitening rinse is only in contact with the teeth for such a short time -- just two minutes a day compared to 30 minutes for many strips -- it may have less of an effect.
The perception of tooth color is the result of a complex interaction of factors such as: lighting conditions, translucency, opacity, light scattering, gloss, the human eye and brain.[1] Teeth are composed of a surface enamel layer, which is whiter and semitransparent, and an underlying dentin layer, which is darker and less transparent. These are calcified, hard tissues comparable to bone. The natural shade of teeth is best considered as such; an off-white, bone-color rather than pure white. Public opinion of what is normal tooth shade tends to be distorted. Portrayals of cosmetically enhanced teeth are common in the media. In one report, the most common tooth shade in the general population ranged from A1 to A3 on the VITA classical A1-D4 shade guide.[2] Gracious Smiles offers dental care to Camden school children
