Extrinsic stains are those that appear on the surface of the teeth as a result of exposure to dark-colored beverages, foods and tobacco, and routine wear and tear. Superficial extrinsic stains are minor and can be removed with brushing and prophylactic dental cleaning. Stubborn extrinsic stains can be removed with more involved efforts, like teeth whitening. Persistent extrinsic stains can penetrate into the dentin and become ingrained if they are not dealt with early.

Dental floss should be used where the teeth are close together, with little or no space between them, while interdental brushes are suitable for larger gaps. Special care should be taken to clean thoroughly around crooked or crowded teeth, or around fillings, crowns and dentures, as plaque accumulates readily in these places and access might be limited.

The term “whitening” on the other hand, refers to restoring a tooth’s surface color by removing dirt and debris. So technically speaking, any product that is used to clean the teeth (like a toothpaste) is considered a whitener. Of course, the term whitening sounds better than bleaching, so it is more frequently used — even when describing products that contain bleach. Emergency Dentist Kensington
You have the choice of two different types of whitening trays in this kit. You get one universal, comfort-fit tray, which is patented exclusively for Cali White, as well as 2 thermaform custom fit trays. The blue LED light fits onto the mouth trays and to make your treatment even easier; it has a built-in timer, so once it’s in you can forget about it until it alerts you that the time has passed. This customization allows for maximum contact between the whitening gel, which is applied to the mouthpiece tray, and the teeth. A custom-made tray also minimizes the gel's contact with gum tissue. The two side effects that occur most often with teeth whitening are a temporary increase in tooth sensitivity and mild irritation of the soft tissues of the mouth, particularly the gums. Tooth sensitivity often occurs during early stages of the bleaching treatment. Tissue irritation most commonly results from an ill-fitting mouthpiece tray rather than the tooth-bleaching agent. Both of these conditions usually are temporary and disappear within 1 to 3 days of stopping or completing treatment. Treatment Options for TMJ / TMD with London Cosmetic Dentist Dr. David Frey
Brushing and flossing are everyday ways to keep your teeth bright, white and healthy. Still, if you might feel like your smile is lacking some sparkle or is more yellow than it used to be, you’re not alone. When the American Academy of Cosmetic Dentistry asked people what they’d most like to improve about their smile, the most common response was whiter teeth. The American Association of Orthodontists also found that nearly 90% of patients requested tooth whitening.
The whitening shade guides are used to measure tooth color. These shades determine the effectiveness of the whitening procedure, which may vary from two to seven shades.[16] The effects of bleaching can last for several months, but may vary depending on the lifestyle of the patient. Consuming tooth staining foods or drinks that have a strong colour may compromise effectiveness of the treatment. These include; coffee, fizzy drinks, red-colored sauces etc. Nervous dental patient review for Bishopsgate Dental Care in London
Messiness aside, the Plus White kit is easy to use. Included are two trays, the whitening gel and a post-whitening rinse. Just brush your teeth with plain water, fill the trays with gel, put them on your teeth for 5 to 20 minutes, then rinse. Users agree with the testers at Reviews.com, saying they noticed whiter teeth pretty quickly, and they became even whiter over the treatment time of 14 days. Dental implant success is related to operator skill, quality and quantity of the bone available at the site, and the patient's oral hygiene, but the most important factor is primary implant stability.[46] While there is significant variation in the rate that implants fail to integrate (due to individual risk factors), the approximate values are 1 to 6 percent[18](p68)[25]
There is no big secret to what makes the best teeth whitening treatment. According to the American Dental Association (ADA) and the dentists we interviewed, hydrogen peroxide and carbamide peroxide are the only things that work. We rounded up the teeth whitening products that cap their peroxide at ADA-recommended levels, then had 39 testers weigh in on their results. Finally, we compared the method of delivery: trays, strips, or a paint-it-on gel to find four whiteners that outperformed the rest. BDS Dental Surgery - Student Experience
Gum disease progresses with the development of plaque over the teeth. This is a kind of bacterial white sticky substance formed when a bacterium mixes itself with saliva along with starch and other ingredients of one's diet called debris. Oral health care is one of the major concerns around the globe as minor dental problems are giving passage to bigger and incurable forms. As a result, how to treat gum disease with homemade remedies is one major worldwide health issue that can help people follow some simple and effective ways for better oral health. Advice to students applying to study dentistry at Manchester
However, later research suggests that the initial stability of the implant in bone is a more important determinant of success of implant integration, rather than a certain period of healing time. As a result, the time allowed to heal is typically based on the density of bone the implant is placed in and the number of implants splinted together, rather than a uniform amount of time. When implants can withstand high torque (35 Ncm) and are splinted to other implants, there are no meaningful differences in long-term implant survival or bone loss between implants loaded immediately, at three months, or at six months.[25] The corollary is that single implants, even in solid bone, require a period of no-load to minimize the risk of initial failure.[26]

A dental implant (also known as an endosseous implant or fixture) is a surgical component that interfaces with the bone of the jaw or skull to support a dental prosthesis such as a crown, bridge, denture, facial prosthesis or to act as an orthodontic anchor. The basis for modern dental implants is a biologic process called osseointegration, in which materials such as titanium form an intimate bond to bone. The implant fixture is first placed so that it is likely to osseointegrate, then a dental prosthetic is added. A variable amount of healing time is required for osseointegration before either the dental prosthetic (a tooth, bridge or denture) is attached to the implant or an abutment is placed which will hold a dental prosthetic. It is also possible to have stains inside the tooth. These are called intrinsic stains. For example, intrinsic stains can be caused by exposure to too much fluoride as a child while teeth are developing. Other causes include tetracycline antibiotics. They can stain a child's teeth if taken by a mother during the second half of pregnancy or by a child who is 8 years old or younger. Teeth are still developing during these years. Trauma may also darken a tooth.

The long-term success of implants is determined, in part, by the forces they have to support. As implants have no periodontal ligament, there is no sensation of pressure when biting so the forces created are higher. To offset this, the location of implants must distribute forces evenly across the prosthetics they support.[18](pp15–39) Concentrated forces can result in fracture of the bridgework, implant components, or loss of bone adjacent the implant.[19] The ultimate location of implants is based on both biologic (bone type, vital structures, health) and mechanical factors. Implants placed in thicker, stronger bone like that found in the front part of the bottom jaw have lower failure rates than implants placed in lower density bone, such as the back part of the upper jaw. People who grind their teeth also increase the force on implants and increase the likelihood of failures.[8](p201–208)
Most teeth whitening products on the market use similar active ingredients to whiten. The difference is the device that is used to hold the whitening agent on the tooth. A truly custom-fitted whitening tray ensures that every angle of every tooth (regardless of whether your teeth are straight or not) receives even and consistent coverage. Unlike boil & bit trays, whitening strips, and UV lights, fitted whitening trays eliminate the possibility of inconsistencies and allow you to maintain your beautiful smile.

Soft tissue grafts. When you lose gum tissue, your gumline recedes. You may need to have some of the damaged soft tissue reinforced. This is usually done by removing a small amount of tissue from the roof of your mouth (palate) or another donor source and attaching it to the affected site. This can help reduce further gum recession, cover exposed roots and give your teeth a more pleasing appearance. Dentist In Holborn London Outstanding 5 Star Review by Federico S.
Dentists and dental hygienists measure periodontal disease using a device called a periodontal probe. This thin "measuring stick" is gently placed into the space between the gums and the teeth, and slipped below the gumline. If the probe can slip more than 3 mm (0.12 in) below the gumline, the person is said to have a gingival pocket if no migration of the epithelial attachment has occurred or a periodontal pocket if apical migration has occurred. This is somewhat of a misnomer, as any depth is, in essence, a pocket, which in turn is defined by its depth, i.e., a 2-mm pocket or a 6-mm pocket. However, pockets are generally accepted as self-cleansable (at home, by the person, with a toothbrush) if they are 3 mm or less in depth. This is important because if a pocket is deeper than 3 mm around the tooth, at-home care will not be sufficient to cleanse the pocket, and professional care should be sought. When the pocket depths reach 6 to 7 mm (0.24 to 0.28 in) in depth, the hand instruments and cavitrons used by the dental professionals may not reach deeply enough into the pocket to clean out the microbial plaque that causes gingival inflammation. In such a situation, the bone or the gums around that tooth should be surgically altered or it will always have inflammation which will likely result in more bone loss around that tooth. An additional way to stop the inflammation would be for the person to receive subgingival antibiotics (such as minocycline) or undergo some form of gingival surgery to access the depths of the pockets and perhaps even change the pocket depths so they become 3 mm or less in depth and can once again be properly cleaned by the person at home with his or her toothbrush.

Nothing replaces your natural teeth, but dental implants can come close. Implants have been used for many years. They are man-made “anchors” that look like screws and are made of titanium and other materials that are compatible with the human body. The single tooth implant is placed by surgery in the upper or lower jaw, where it replaces your missing tooth’s root(s). Cost of Dental Implants HIDDEN DANGERS
Our testers agreed it was tough to tell how much they were using and how much was left in the pen, since the plastic casing isn’t clear. Because the gel itself is clear and relatively flavorless, it might be difficult to tell if you’re using enough product to cover your teeth or if you’re spreading it evenly. But careful, even application should prevent this from being too much of an issue. Best Cosmetic Dentist London - My Experience
One way to address staining is by using at-home teeth-whitening treatment kits. These products are milder versions of the bleaching kits that are used by dentists to whiten teeth in the office. Teeth-whitening treatments typically use either hydrogen peroxide or carbamide peroxide as an active ingredient. When the whitening gel or liquid is applied to the teeth, the key ingredient creates a chemical reaction, helping to break down the stain. Typically, teeth-whitening treatment products must be left in place for a set number of minutes daily for a certain period of time, such as one or two weeks. You also can buy whitening products over the counter. They contain a weaker whitening agent than the products you can get from your dentist. Therefore, whitening may take longer. The whitening agent is applied as a gel placed in a mouthpiece or as a strip that sticks to your teeth. Over-the-counter mouthpieces fit less securely than the kind you get from your dentist.

Typically, dental hygienists (or dentists) use special instruments to clean (debride) teeth below the gumline and disrupt any plaque growing below the gumline. This is a standard treatment to prevent any further progress of established periodontitis. Studies show that after such a professional cleaning (periodontal debridement), microbial plaque tends to grow back to precleaning levels after about three to four months. Nonetheless, the continued stabilization of a person's periodontal state depends largely, if not primarily, on the person's oral hygiene at home, as well as on the go. Without daily oral hygiene, periodontal disease will not be overcome, especially if the person has a history of extensive periodontal disease.[citation needed] Dental Implants - Brightside Dental - Dentist North London
The damage to teeth and gums comes from the immune system as it attempts to destroy the microbes that are disrupting the normal symbiosis between the oral tissues and the oral microbe community. As in other tissues, Langerhans cells in the epithelium take up antigens from the microbes, and present them to the immune system, leading to movement of white blood cells into the affected tissues. This process in turn activates osteoclasts which begin to destroy bone, and it activates matrix metalloproteinases that destroy ligaments.[46] So, in summary, it is bacteria which initiates the disease, but key destructive events are brought about by the exaggerated response from the host's immune system.[40]

The word "periodontitis" (Greek: περιοδοντίτις) comes from the Greek peri, "around", odous (GEN odontos), "tooth", and the suffix -itis, in medical terminology "inflammation".[69] The word pyorrhea (alternative spelling: pyorrhoea) comes from the Greek pyorrhoia (πυόρροια), "discharge of matter", itself from pyon, "discharge from a sore", rhoē, "flow", and the suffix -ia.[70] In English this term can describe, as in Greek, any discharge of pus; i.e. it is not restricted to these diseases of the teeth.[71]

For example, did you know implants can help prevent bone loss in the jaw? The natural erosion of bone begins as soon as a tooth is lost, and if nothing is done, this may result in the loss of other teeth and could cause a “sunken” look in facial features. Dental implants integrate with the bone, providing a stimulus that keeps the jawbone from deteriorating. This helps maintain a more youthful facial structure, boosts confidence—and leads to better oral health.

Sinus Augmentation   A key to implant success is the quantity and quality of the bone where the implant is to be placed. The upper back jaw has traditionally been one of the most difficult areas to successfully place dental implants due to insufficient bone quantity and quality and the close proximity to the sinus. Sinus augmentation can help correct this problem by raising the sinus floor and developing bone for the placement of dental implants. About our dental practice - Bishopsgate Dental Care in London

Dental implants are artificial tooth roots that provide a permanent base for fixed, replacement teeth. Compared to dentures, bridges and crowns, dental implants are a popular and effective long-term solution for people who suffer from missing teeth, failing teeth or chronic dental problems. Because they fit, feel and function like natural teeth, dental implants are quickly becoming the new standard in tooth replacement.
For minor discoloration that is confined to one or two teeth, you can try using a teeth-whitening pen. A tooth-whitening pen uses the same types of ingredients present in a teeth-whitening treatment but allows you to precisely apply the bleach to the stain. Like teeth-whitening kits, pens must be used repeatedly over an extended period of time to produce noticeable results.

Flap surgery/pocket reduction surgery. During this procedure the gums are lifted back and the tarter is removed. In some cases, irregular surfaces of the damaged bone are smoothed to limit areas where disease-causing bacteria can hide. The gums are then placed so that the tissue fits snugly around the tooth. This method reduces the size of the space between the gum and tooth, thereby decreasing the areas where harmful bacteria can grow and decreasing the chance of serious health problems associated with periodontal disease.

Unfortunately, gum disease does not usually cause pain as it gets worse so you do not notice the damage it is doing. However, the bacteria are sometimes more active and this makes your gums sore. This can lead to gum abscesses, and pus may ooze from around the teeth. Over a number of years, the bone supporting the teeth can be lost. If the disease is left untreated for a long time, treatment can be more difficult. Visiting the Eastman Dental Hospital

At-home whitening methods include gels, chewing gums, rinses, toothpastes, paint-on films, and whitening strips.[22][23] Most over-the-counter methods utilize either carbamide peroxide or hydrogen peroxide.[23] Although there is some evidence that such products will whiten the teeth compared to placebo, the majority of the published scientific studies were short term and are subject to a high risk of bias as the research was sponsored or conducted by the manufacturers.[23] There is no long term evidence of the effectiveness or potential risks of such products.[23] Any demonstrable difference in the short term efficiency of such products seems to be related to concentration of the active ingredient.[23]

Scaling and root planing. This is a deep-cleaning, nonsurgical procedure, done under a local anesthetic, whereby plaque and tartar from above and below the gum line are scraped away (scaling) and rough spots on the tooth root are made smooth (planing). Smoothing the rough spots removes bacteria and provides a clean surface for the gums to reattach to the teeth. Scaling and root planing is done if your dentist or periodontist determines that you have plaque and calculus (hardened plaque, also called tartar) under the gums that needs to be removed.
In the early stages of periodontitis, your gums recede, or pull away, from your teeth and small pockets form between gums and teeth. The pockets harbor harmful bacteria. Your immune system tries to fight the infection, and your gum tissue starts to recede. You’ll likely experience bleeding during brushing and flossing as well, and possibly some bone loss.

The general recommendation is to use over-the-counter whitening products up to two times a year. Using whitening products too often can lead to increased tooth sensitivity or white spots — this is natural and due to certain areas of your teeth that have a lack of calcium (also known as hypocalcification). Excessive whitening can make the spots more prominent. Cosmetic Dentistry - London Cosmetic Dentists londonsmile.co.uk Video

The more dangerous bacteria are able to thrive and multiply, producing some harmful by-products which stimulate the body’s defensive inflammatory response in the gums. As the disease progresses, chronic inflammation causes the bone of the jaw to be destroyed and the teeth to be lost. In many people, this is a gradual process that takes place over many years and, if detected and treated, can be halted. As with most treatment procedures in dentistry today, dental implants not only involve scientific discovery, research and understanding, but also application in clinical practice. The practice of implant dentistry requires expertise in planning, surgery and tooth restoration; it is as much about art and experience as it is about science. This site will help provide you with the knowledge you need to make informed choices in consultation with your dental health professionals. Risks: I’m not a fan of these light systems and won’t use them in my practice. At best, they’re safe, but they won’t come close to achieving the results of custom trays or whitestrips. At worst, accelerating the chemical reaction damages the tooth. This can lead to premature aging and yellowing and may require future dental work if the tooth dies prematurely. Periodontists are dentists who have completed additional training after dental school focused entirely on the evaluation, diagnosis, and treatment of disease of the gum and bone surrounding the teeth. In advanced cases of gingivitis or periodontal disease, a general dentist may recommend a gross debridement of superficial plaque and tartar and will then refer the affected patient to a periodontist for evaluation. Periodontists may recommend a variety of nonsurgical or surgical procedures to help stabilize the gum condition. Periodontists are also very skilled at performing treatments of other conditions of the gums and bone including gum and bone grafts, functional and esthetic gingivectomy, gingivoplasty, implants, and crown lengthening.
The AARP Bulletin warned that dental disease has become a silent epidemic among older Americans.  Older adults with missing teeth often have underlying periodontal disease from which bacteria from chronic gum infection can migrate through the bloodstream to vital organs.  Adults with missing teeth are unable to eat properly and often develop nutritional deficiencies from an inadequate or unhealthy diet.
The prosthetic phase begins once the implant is well integrated (or has a reasonable assurance that it will integrate) and an abutment is in place to bring it through the mucosa. Even in the event of early loading (less than 3 months), many practitioners will place temporary teeth until osseointegration is confirmed. The prosthetic phase of restoring an implant requires an equal amount of technical expertise as the surgical because of the biomechanical considerations, especially when multiple teeth are to be restored. The dentist will work to restore the vertical dimension of occlusion, the esthetics of the smile, and the structural integrity of the teeth to evenly distribute the forces of the implants.[8](pp241–251) Refresh Dental a Dental Clinic in London for Cosmetic Dentistry or for Dentist

The cornerstone of successful periodontal treatment starts with establishing excellent oral hygiene. This includes twice-daily brushing with daily flossing. Also, the use of an interdental brush is helpful if space between the teeth allows. For smaller spaces, products such as narrow picks with soft rubber bristles provide excellent manual cleaning. Persons with dexterity problems, such as arthritis, may find oral hygiene to be difficult and may require more frequent professional care and/or the use of a powered toothbrush. Persons with periodontitis must realize it is a chronic inflammatory disease and a lifelong regimen of excellent hygiene and professional maintenance care with a dentist/hygienist or periodontist is required to maintain affected teeth. In-office bleaching provides the quickest way to whiten teeth. With in-office bleaching, the whitening product is applied directly to the teeth. These products can be used in combination with heat, a special light, or a laser. Results are seen in only one, 30- to 60-minute treatment. But to achieve dramatic results, several appointments are usually needed. However, with in-office bleaching, dramatic results can be seen after the first treatment. This type of whitening is also the most expensive approach.
Once you have applied the whitening gel, either in strips or tray form, you can turn on the blue LED light and hold it against your teeth. Some kits have a light you can bite down on so that you don’t have to hold it in place, while others require you to sit still and hold the light in place as it works to cure the whitening gel. Most teeth whitening kits have LED lights that provide up to 460 nanometers of light, but it will vary from kit to kit. The light doesn’t just help to cure the whitening gel; it actually helps activate the ingredients in it to provide the whitest smile possible. Sometimes, a surgical procedure is carried out to clean away plaque bacteria and deposits that are under the gum within periodontal pockets and on the root surfaces at the furcations (where the roots diverge). These areas are inaccessible to brushes and floss and inflammation will persist in these sites as long as bacteria are allowed to colonize them. Under local anaesthesia, the gum is lifted away and the root surfaces are cleaned under direct vision to ensure that all bacteria are removed. Sometimes, it is possible to treat bone loss at the same time using a special regenerative treatment. At the end of the procedure, the gums are sutured back into place around the teeth.

Supervised vs. unsupervised process. First, your dentist can perform an oral exam and consider your complete medical history, which can be helpful in determining if bleaching is an appropriate course of treatment based on the type and extent of stains, and the number and location of restorations. Your dentist can then better match the type of stain with the best treatment, if appropriate, to lighten those stains.

Bruxism (clenching and grinding of the teeth) can lead to micro-cracking of the incisal edges of the teeth. Extrinsic stains may settle more readily into these cracks, and a thin layer of enamel can be left. This thin enamel layer is partially transparent, allowing the dark background of the mouth to shine through, affording a darker appearance of the incisal edge. Tooth whitening toothpastes. Because they're mildly abrasive, every toothpaste helps remove stains from teeth. Whitening toothpastes, however, also contain chemicals or polishing agents that help scrub stains from teeth without the aid of a bleaching agent. Tooth-whitening toothpastes are relatively inexpensive and brighten teeth by about one shade. Some whitening toothpastes contain peroxides, but they aren't left on the teeth long enough to have a whitening benefit. What's in that teeth whitening product? We wanted to include the percentage of whitening ingredient in each of the teeth whitening products that we recommend, but it's not easy to ascertain that number because the manufacturers do not include those specifics on their websites. In each case, we reached out to the manufacturer to verify ingredient percentages, with mixed results. Some makers were glad to share percentages, others politely declined, saying that was considered proprietary information.
